New Patient Information

 Therapy Care Plans: Your therapist will discuss your care plan with you after your initial office visit. It is important that you adhere to the program outlined by the therapist for the duration of your treatment. In general, you will need treatment 2 to 3 times per week for several weeks as ordered by your physician. Please wear loose, comfortable clothing to your therapy sessions.

New Patient Forms
Your first visit requires the therapist to perform an initial evaluation, which requires more time than the subsequent visits. You can help maximize your time with the therapist by expediting the admissions process. Please print and complete the following forms prior to your first visit and bring them with you, along with a copy of your driver's license and insurance card. Completing the forms prior to your first visit will help ensure that you receive the maximum amount of personalized attention with your therapist.
